Клиническая картина заболевания и обстоятельства жизни человека с ВДИ, выводят данную категорию пациентов в разряд особо уязвимой с точки зрения развития психических нарушений, а уже возникшие проблемы ментального характера по принципу «порочного круга» могут усугублять физическое состояние, повышать частоту госпитализаций, создавая дополнительную нагрузку на систему здравоохранения. Пациент с уже диагностированным ВДИ нуждается в пожизненной заместительной терапии иммуноглобулином, что создает дополнительные специфические трудности ятрогенного и социально-психологического характера: регулярное посещение медицинских учреждений, многократные инфузионные процедуры, «игольная фобия», зависимость жизни от медикаментозного сопровождения, «лекарственная тревога» и др. Важно отметить, что хронические стрессовые состояния, в свою очередь, часто приводят к значимой иммуносупрессии, что подчеркивает важность учета их роли в развитии психических и ухудшении иммунных расстройств у пациентов с ВДИ. В свою очередь, нарушения в работе иммунной системы могут играть этиологическую роль в развитии психических расстройств посредством постинфекционной активации микроглии и микробного дисбиоза. В Российской Федерации (РФ) среднее время от появления симптомов до установления диагноза ВДИ приблизительно составляет 17 лет, что за такой продолжительный период времени приводит не только к существенному ухудшению здоровья пациента, но и к комплексному снижению качества жизни, социального функционирования, к утрате трудоспособности и другим последствиям психологического характера. Целью данной работы являлось проведение анализа накопленных отечественных и зарубежных научных публикаций по вопросам психологического благополучия пациентов с ВДИ для определения роли психологического сопровождения в комплексной терапии взрослых с врожденными дефектами иммунитета (первичными иммунодефицитами) и основных направлений в работе медицинского психолога с данной группой пациентов. Представлен обзор психосоциальных аспектов жизни пациентов с ВДИ, исследований по вопросам имеющихся у них психических нарушений, удовлетворенности качеством жизни. Рассмотрены основные принципы и возможные направления в работе медицинского психолога с пациентами, страдающими ВДИ, при соблюдении которых будет реализована системная и последовательная работа, индивидуальный подход с учетом особенностей каждого отдельного пациента, а также долгосрочный характер интервенций. По результатам проведенной работы сделан вывод о том, что специфические психосоциальные факторы заболевания, а также тесная взаимосвязь функционирования психики и иммунной системы человека создают уникальный психологический профиль пациентов c ВДИ, требующий описания и уточнения в рамках мультидисциплинарных научных исследований.</p></abstract><trans-abstract xml:lang="en"><p>Inborn errors of immunity (IEI) are severe, genetically determined, chronic diseases resulting from the loss, reduction, or malfunction of parts of the immune system. The clinical presentation and lifestyle circumstances of individuals with IEI make this category of patients particularly vulnerable to the development of mental disorders. Once mental health problems have developed, they can, in a vicious cycle, worsen physical conditions, increase hospitalization rates, and place additional strain on the healthcare system. A patient diagnosed with IEI requires lifelong immunoglobulin replacement therapy, which creates additional specific iatrogenic and sociopsychological challenges: regular medical visits, multiple infusion procedures, "needle phobia," dependence on medication, "medication anxiety," etc. Furthermore, immune system dysfunction can play an etiological role in the development of mental disorders through post-infectious microglial activation and microbial dysbiosis. In the Russian Federation, the average time from the onset of symptoms to diagnosis of IEI is approximately 17 years. This prolonged period of time not only leads to a significant deterioration in the patient's health but also to a comprehensive decline in quality of life. The aim of this study was to analyze accumulated domestic and international scientific publications on the psychological well-being of patients with inborn errors of immunity to determine the role of psychological support in the comprehensive treatment of adults with Inborn errors of immunity (primary immunodeficiencies) and the main areas of clinical psychologist work with this group of patients. This study provides an overview of the psychosocial aspects of the lives of patients with IEI, as well as research on their mental disorders and satisfaction with their quality of life. The study also examines the key principles and possible areas of clinical psychologist work with patients with IEI. Adherence to these principles will enable systematic and consistent work, an individualized approach tailored to each patient's needs, and long-term interventions.
