Выполнен сравнительный анализ двух клинических групп: 41 пациент со стадией IV, низкой степенью дифференцировки, множественным метастатическим поражением и выявляемыми циркулирующими опухолевыми клетками и циркулирующей опухолевой ДНК; 41 пациент со стадией I без признаков циркулирующего опухолевого материала в периферической крови. Оценивали воспалительные маркеры, включая фактор некроза опухоли альфа, интерлейкин-1 бета, интерлейкин-6, C-реактивный белок, сывороточный амилоид A, ферритин, скорость оседания эритроцитов и лейкоциты. Нейроэндокринный блок включал мелатонин, дофамин, серотонин, адреналин, норадреналин, кортизол и гистамин. Опухолевый блок включал цитокератиновый фрагмент CYFRA 21-1, карциноэмбриональный антиген, циркулирующие опухолевые клетки и циркулирующую опухолевую ДНК. В метастатической группе отмечалось согласованное повышение провоспалительных показателей, наиболее выраженное для фактора некроза опухоли альфа и интерлейкина-6, что отражает активацию системного воспаления. Одновременно было выявлено резкое снижение мелатонина и дофамина на фоне умеренного повышения кортизола, гистамина, серотонина и катехоламинов. Наиболее выраженные изменения среди опухолевых маркеров были характерны для циркулирующих опухолевых клеток и циркулирующей опухолевой ДНК. Полученные данные поддерживают концепцию, согласно которой прогрессирование аденокарциномы легкого сопровождается не только ростом опухолевой массы, но и перестройкой системного ответа организма. Комбинация цитокинов, нейроэндокринных медиаторов и жидкостной биопсии может рассматриваться как интегральная панель опухоль-хозяин для стратификации риска, динамического наблюдения и планирования проспективных иммунологических исследований. Работа имеет предварительный характер и требует подтверждения в более крупной когорте с индивидуальными количественными данными, стандартизированным временем забора крови и расширенным статистическим анализом.</p></abstract><trans-abstract xml:lang="en"><p>The aim of this study was to evaluate an integrated inflammatory, neuroendocrine and tumor-associated biomarker profile in patients with metastatic lung adenocarcinoma compared with patients with early-stage disease. A comparative analysis was performed in two clinical groups: 41 patients with stage IV poorly differentiated non-small cell lung cancer adenocarcinoma, multiple metastatic lesions and detectable circulating tumor cells and circulating tumor DNA; and 41 patients with stage I disease without detectable circulating tumor material in peripheral blood. The inflammatory panel included tumor necrosis factor alpha, interleukin-1 beta, interleukin-6, C-reactive protein, serum amyloid A, ferritin, erythrocyte sedimentation rate and leukocytes. The neuroendocrine panel included melatonin, dopamine, serotonin, adrenalin, noradrenalin, cortisol and histamine. The tumor-associated panel included CYFRA 21-1, carcinoembryonic antigen, circulating tumor cells and circulating tumor DNA. The metastatic group demonstrated a coordinated increase in inflammatory markers, most prominently tumor necrosis factor alpha and interleukin-6, indicating activation of systemic inflammatory signaling. At the same time, a marked decrease in melatonin and dopamine was observed together with moderate increases in cortisol, histamine, serotonin and catecholamines. The strongest tumor-associated changes were detected for circulating tumor cells and circulating tumor DNA, supporting active tumor shedding and a high metastatic burden. These findings support the concept that progression of lung adenocarcinoma is accompanied not only by expansion of tumor mass, but also by remodeling of the systemic host response. The combined assessment of cytokines, neuroendocrine mediators and liquid-biopsy markers may represent an integrated tumor-host panel for risk stratification, follow-up planning and prospective immunological research. This work should be interpreted as a preliminary short communication.
