Семейная средиземноморская лихорадка (ССЛ) является аутовоспалительным заболеванием (АВЗ), ассоциированным с патогенными вариантами гена MEFV. ССЛ считается моногенным АВЗ с аутосомно-рецессивным механизмом наследования: заболевание развивается у пациентов с двумя патогенными вариантами в транс-положении, а лица с одним патогенным вариантом – носители. Однако в литературе встречаются описания клинических случаев ССЛ у гетерозиготных носителей. К тому же клиническая картина варьирует в зависимости от обнаруженных вариантов.</p></sec><sec><title>Цель</title><p>Цель. Исследовать особенности цитокинового профиля у пациентов с разными генотипами гена MEFV для оценки агрессивности выявленных генетических вариантов и генотипов.</p></sec><sec><title>Материалы и методы</title><p>Материалы и методы. В исследование были включены 275 пациентов, обратившихся для поиска патогенных вариантов в гене MEFV. Для всех обследованных было проведено секвенирование 2, 3, 5 и 10 экзонов гена MEFV методом секвенирования по Сэнгеру. Исследование уровня цитокинов (СРБ, IL-1, IL-6, IL-8 и IL-18) проводилось в образцах ЭДТА-плазмы, полученной из венозной крови центрифугированием и однократно замороженной при температуре -80°C. В качестве группы сравнения были выбраны образцы 24 условно здоровых доноров из биобанка лаборатории. Для определения уровня цитокинов использовались наборы производителя Вектор-БЕСТ, Россия. Статистическая обработка проведена с использованием программы GraphPadPrism 8 (GraphPadSoftwareInc., США).</p></sec><sec><title>Результаты</title><p>Результаты. По результатам генотипирования 275 образцов были разделены на подгруппы в зависимости от выявленного генотипа: нормальный генотип – WT (n=60); гомозиготы (n=28), компаунд-гетерозиготы (n=71), гетерозиготы (n=86), носители патогенных вариантов в сочетании с вариантами неясной клинической значимости (VUS) (n=15) – составили группу FMF; носители только VUS (n=15) были исключены из дальнейшего исследования.</p><p>При исследовании уровня цитокинов методом иммуноферментного анализа в 260 образцах ЭДТА-плазмы (группы FMF, n=200, и WT, n=60) и 24 образцах сыворотки здоровых доноров (HD, n=24) отмечалось повышение уровня СРБ, IL-1, IL-6, IL-18 в группе FMF в сравнении с группой HD (для всех цитокинов P&lt;0,0001), а также отмечалось увеличение уровня IL-6 (P&lt;0,05) и IL-18 (P&lt;0,0001) в группе FMF в сравнении с группой WT, различий уровня IL-8 обнаружено не было. Выявлены более высокие значения уровня СРБ (P&lt;0,01) и IL-18 (P&lt;0,0001) в группе гомозигот с обнаруженными агрессивными вариантами в сравнении с гомозиготными неагрессивными вариантами, а также уровень IL-18 был выше в подгруппе агрессивных гетерозиготных вариантов в сравнении с неагрессивными (P&lt;0,05).</p></sec><sec><title>Выводы</title><p>Выводы. Генотип и агрессивность обнаруженных вариантов оказывают влияние на активность воспалительного процесса у пациентов с ССЛ.</p></sec></abstract><trans-abstract xml:lang="en"><sec><title>Introduction</title><p>Introduction. Familial Mediterranean fever (FMF) is a monogenic autosomal recessive autoinflammatory disease caused by MEFV gene pathogenic variants. Heterozygosity remains a clinically challenging issue: while two variants are classically required for disease manifestation, individuals with one pathogenic variant are only carriers. However, there are a lot of cases of FMF clinical picture in heterozygous carriers of MEFV variants.</p></sec><sec><title>Aim</title><p>Aim. To study the features of the cytokine profile in patients with detected MEFV variants, taking into account molecular-genetic features such as zygosity and aggressiveness of pathogenic variants.</p></sec><sec><title>Materials and Methods</title><p>Materials and Methods. The study included 275 patients referred for MEFV genetic testing. Genomic DNA and EDTA-plasma were obtained from all subjects. Sanger sequencing of exons 2, 3, 5, and 10 of the MEFV gene was performed. Plasma levels of CRP, IL‑1β, IL‑6, IL‑8, and IL‑18 were measured by ELISA. A control group comprised 24 healthy donors from the laboratory biobank. Statistical analysis was carried out using GraphPad Prism 8.</p></sec><sec><title>Result</title><p>Result. Based on genotyping results 275 samples were divided into groups: wild-type genotype – WT (n=60); FMF group includes homozygous (n=28), compound heterozygous (n=71), heterozygous (n=86), carriers of combination of pathogenic and variants of unknown significance (VUS) (n=15); for 15 samples of only VUS carriers’ cytokines’ levels were not measured. In the result of ELISA was shown an increase of CRP, IL-1, IL-6 and IL-18 levels in FMF compared to HD (P&lt;0.0001), as well as an increase of IL-6 (P&lt;0.05) and IL-18 (P&lt;0.0001) was found in FMF compared to WT. There was shown no differences in IL-8 level between FMF, WT and HD groups. An increased levels of CRP (P&lt;0.01) and IL-18 (P&lt;0.0001) were shown in the subgroups with aggressive homozygous variants compared to low-aggressive ones, as well as an increased IL-18 level was obtained in aggressive heterozygous compared to low-aggressive ones (P&lt;0.05). An increased level of IL-1β (P&lt;0.05) was shown in the subgroup with two aggressive pathogenic variants compared to subgroup with one aggressive and one low-aggressive pathogenic variant.</p></sec><sec><title>Conclusion</title><p>Conclusion.
