Хронические риносинуситы (ХРС) представляет собой серьезную медико-социальную проблему, обусловленную высокой частотой встречаемости среди населения земного шара. По данным литературы, примерно 5–12% жителей планеты подвержены данному заболеванию. Особо сложными патогенетическими вариантами ХРС являются полипозный риносинусит (ПРС) и хронический гиперпластический риносинусит (ХГРС).</p><p>Выраженное снижение качества жизни, длительные периоды нетрудоспособности и распространенность этих патологий актуализируют дальнейшие исследования их патогенеза. Большинство фенотипов ПРС протекает по Т2 типу, однако есть   Т1 и Т3 и смешанные варианты. Для ХГРС не установлен доминирующий эндотип. </p><p>Полагаем, что у пациентов при ПРС и ХГРС будут отличия в иммунном ответе. Набор цитокинов и особенности иммунного ответа могут различаться, оказывая специфическое влияние на разрастание слизистой оболочки носовой полости и придаточных пазух носа.</p><p>       Цель работы - изучение особенностей цитокиновой регуляции при полипозном риносинусите и хроническом гиперпластическом риносинусите.</p><p>       Материалы и методы. Обследовано 68 человек с риносинуситами и 43 здоровых человека. В исследование приняли участие пациенты с полипозным риносинуситом (ПРС, n=33), хроническим гиперпластическом риносинусите (ХГРС, n=35) и группа контроля (n=43), состоящая из практически здоровых. Тип исследования – «случай-контроль» выполнено в лаборатории клинической патофизиологии НИИ МПС. Материалы для исследования –сыворотка крови и назальное отделяемое. Содержание IFN-γ, IL-1β, IL-2, IL-4, IL-5, IL-8, IL-10, IL-13, IL-17А определяли метом ИФА, используя с реагенты «Вектор-Бест» (г. Новосибирск) и спектрофотометр Muitiskan FC. Статистическая обработка проведена с помощью программы Statistica 10.0. Описание выборки с подсчетом медианы и интерквартильного размаха 25 и 75 перцентилей, критерий Манна–Уитни, уровень значимости гипотезы менее 0,05.</p></sec><sec><title>           Результаты</title><p>           Результаты. </p><p>Исследование концентраций цитокинов показало достоверные межгрупповые отличия на местном и системном уровне. У пациентов группы контроля обнаружены низкие значения всех исследуемых цитокинов системно и локально. Определены однонаправленные изменения для каждого фенотипа ХРС. Так, для ПРС локально и системно обнаружены высокие концентрации IL-4, IL-5 и IL-13 относительно группы контроля и группы ХГРС. Для ХГРС локально и системно определено увеличение содержания IFN-γ, IL-2, IL-10, IL-17А, IL-8 и IL-1β относительно контроля и ПРС.</p></sec><sec><title>         Выводы</title><p>         Выводы. </p><p>Для ПРС локально и системно определен Т2 тип иммунного ответа, а для ХГРС локально и системно показан смешанный тип Т1/Т3 иммунного ответа</p></sec></abstract><trans-abstract xml:lang="en"><sec><title>Background</title><p>Background. Chronic rhinosinusitis (CRS) is a serious medical and social problem due to its high prevalence worldwide. According to the literature, approximately 5–12% of the world's population is susceptible to this disease. Particularly complex pathogenetic variants of CRS include polypous rhinosinusitis (PRS) and chronic hyperplastic rhinosinusitis (CHRS).</p><p>The significant reduction in quality of life, prolonged periods of disability, and the prevalence of these pathologies necessitate further research into their pathogenesis. Most PRS phenotypes follow the T2 type, but there are also T1, T3, and mixed variants. A dominant endotype has not been established for CHRS.</p><p>We believe that patients with PRS and CHRS will have different immune responses. Cytokine profiles and immune response characteristics may vary, specifically influencing mucosal proliferation in the nasal cavity and paranasal sinuses.</p><p>The aim of this study was to investigate cytokine regulation in polypous rhinosinusitis and chronic hyperplastic rhinosinusitis.</p></sec><sec><title>Materials and Methods</title><p>Materials and Methods: 68 individuals with rhinosinusitis and 43 healthy individuals were examined. The study included patients with polypous rhinosinusitis (PRS, n=33), chronic hyperplastic rhinosinusitis (CHRS, n=35), and a control group (n=43) consisting of apparently healthy individuals. This case-control study was conducted in the Clinical Pathophysiology Laboratory of the Research Institute of the Ministry of Railways. Blood serum and nasal secretions were used for the study. Levels of IFN-γ, IL-1β, IL-2, IL-4, IL-5, IL-8, IL-10, IL-13, and IL-17A were determined by ELISA using Vector-Best reagents (Novosibirsk) and a Muitiskan FC spectrophotometer. Statistical analysis was performed using Statistica 10.0. Sample description included calculation of the median and interquartile range (IQR) at the 25th and 75th percentiles, the Mann-Whitney test, and a significance level of &lt;0.05.</p></sec><sec><title>Results</title><p>Results.</p><p>A study of cytokine concentrations revealed significant intergroup differences at the local and systemic levels. Patients in the control group had low levels of all studied cytokines, both locally and systemically. Consistent changes were identified for each CRS phenotype. Thus, high concentrations of IL-4, IL-5, and IL-13 were detected locally and systemically in PRS compared to the control group and the chronic hyperplastic rhinosinusitis group.
