Гипертрофическое нарастание мышечной массы при ЛМ сопровождается развитием сосудистых сетей, которые регулируется балансом проангиогенных факторов, одним из которых является VEGF-A. Кроме того, проангиогенным эффектом обладает ряд воспалительных молекул, наиболее значимые из которых IL-1β, IL-8 и др. Спектр их активности может перекрываться и регулироваться другими цитокинами. Цель – оценить уровень концентрации цитокинов, принимающих активное участие в регуляции роста сосудистой сети в периферической крови пациенток с множественной миомой матки в сопоставлении с данными по группе условно здоровых женщин.</p><p>В обследование вошли 178 женщин. Из них 89 женщин (23-60 лет) с ЛМ и 89 условно здоровых женщин аналогичного возраста (22-61 год). Уровни TNFα, IL-1β, IL-4, IL-6, IL-8, IL-10 и VEGF-A определялся методом ИФА (АО «Вектор-Бест», Россия). Статистическая обработка результатов проводилась с использованием пакета прикладных программ IBM SPSS Statistics 23 (США).</p><p>Концентрации IL-6, TNFα и IL-8 в сыворотке крови выше среди пациенток при сравнении со здоровыми женщинами. Установлена достоверная зависимость содержания VEGF от количества миоматозных узлов. При множественных выявленных узлах уровень VEGF выше. У здоровых женщин повышение концентрации TNFα, прямо коррелирует с повышением сывороточных концентраций IL-6. Корреляция с концентрацией VEGF носит слабо отрицательный характер. При лейомиоме характер этих взаимосвязей сохраняется: для IL-6, IL-8, VEGF. Для всех пациенток картина корреляционных отношений остается неизменной.</p><p>Полученные данные имеют практическое значение не только в качестве основы для разработки прогностических критериев развития ЛМ на доклиническом этапе, но и в качестве дифференциально диагностических дополнительных лабораторных признаков. В частности, при дифференциации лейомиомы матки, наиболее частой доброкачественной миоматозной опухоли матки, от злокачественных лейомиосарком матки.</p></abstract><trans-abstract xml:lang="en"><p>Uterine leiomyomas (UL) are benign uterine tumors. Hypertrophic increase in muscle mass in LM is accompanied by development of vascular networks, which are regulated by the balance of pro-angiogenic factors, e.g., VEGF-A. Moreover, a number of inflammatory molecules exert pro-angiogenic effects, especially, IL-1β, IL-8, etc. The spectrum of their activity may overlap, being regulated by other cytokines. The aim of our work was to assess serum concentrations of cytokines actively involved in vascular network growth in the patients with multiple uterine fibroids, as compared with data obtained in conditionally healthy women. The survey included 178 females: 89 women (23-60 years old) with uterine fibroids, and 89 conditionally healthy age-matched women (22-61 years old). The levels of TNFα, IL-1β, IL-4, IL-6, IL-8, IL-10 and VEGF-A were detected by ELISA technique (Vector-Best, Russia). Statistical analysis was carried out using the IBM SPSS Statistics 23 (USA). Serum levels of IL-6, TNF and IL-8, were higher among patients compared with healthy women. The dependence of VEGF level on the number of myoma nodes has been established: VEGF serum level was higher in patients with multiple tumor nodes. In healthy women, an increase in TNFα level showed direct correlation with higher serum level of IL-6. Correlation with VEGF level was weakly negative. In leiomyoma, these relationships persist for IL-6, IL-8, VEGF levels.
