Это системное воспалительное гранулематозное заболевание неизвестной этиологии, характеризующееся образованием в пораженных тканях эпителиоидноклеточных гранулем. Интенсивность развития воспалительного ответа может зависеть от многих факторов, в том числе от генетического фона организма. Генетический фон может определять не только восприимчивость людей к возникновению саркоидоза легких, но также клинические характеристики протекания данного заболевания и силу развития воспалительных реакций со стороны иммунной системы. В числе генетических факторов может выступать аллельный полиморфизм генов. У носителей определенных аллельных вариаций генов можно наблюдать либо увеличение, либо уменьшение продукции провоспалительных факторов. Среди генов-кандидатов, чьи продукты могут быть вовлечены в восприимчивость людей к формированию гранулемы — гены, кодирующие Toll-подобные рецепторы (Toll-like receptors, TLRs). Сведения о связи носительства аллельных вариаций указанных генов с восприимчивостью к саркоидозу легких, а также вклад полиморфных вариантов этих генов в развитие, прогрессирование и терапию данного заболевания еще весьма малочисленны и зачастую противоречивы.</p><p>Цель исследования заключалась в изучении связи полиморфизма Arg753Gln (rs574308) гена Toll-подобного рецептора 2 (TLR2) с риском развития саркоидоза легких.</p><p>Обследовано 253 человека (122 больных русской национальности (проживающих в Республике Карелия)) с диагнозом «морфологически верифицированный саркоидоз с поражением легких» (ср. возраст — 41,00±12,56 года) и 131 здоровый донор (контрольная группа) (ср. возраст — 44,00±14,23 года). Исследовано распределение аллелей и генотипов по полиморфному маркеру Arg753Gln (rs574308) гена TLR2 в группе больных саркоидозом легких и в группе здоровых доноров. Идентификацию аллелей данного полиморфного маркера проводили методом полимеразной цепной реакции с последующим анализом длин рестрикционных фрагментов (ПЦР-ПДРФ метод).</p><p>Статистически значимых различий в распределении частот аллелей и генотипов полиморфному маркеру Arg753Gln (rs574308) гена TLR2 между контрольной группой и группой больных саркоидозом легких не выявлено: χ2 = 2,0, df = 1, p = 0,158 и χ2 = 2,19, df = 2, p = 0,140 соответственно.</p><p>Полиморфный маркер Arg753Gln (rs574308) гена Toll-подобного рецептора 2 не связан с риском развития саркоидоза легких у русского населения Республики Карелия.</p></abstract><trans-abstract xml:lang="en"><p>Genetic factors play an important role in the development and progression of many disorders including lung sarcoidosis which is a systemic inflammatory granulomatous disease of unknown etiology, characterized by the formation of epithelioid cell granules in affected tissues. Intensity of the developing inflammation may partially depend on genetic factors which may influence both susceptibility to lung sarcoidosis, and also clinical course of the disease and the degree of inflammatory response from the immune system. Allelic polymorphism of distinct genes is therefore worth of study. In the carriers of certain allele variants, one may observe either increase, or a decreased production of pro-inflammatory factors. Among the candidate factors involved in higher susceptibility of humans, one may consider Toll-like receptors (TLRS) which may contribute to formation of granulomas. Relevant data concerning association between the allele variants of these genes and susceptibility to lung sarcoidosis, and its clinical course are still quite limited and contradictory. The aim of the present study was to analyze the association between the Arg753Gln (rs574308) polymorphism of the Toll-like receptor 2 (TLR2) gene and the risk of developing pulmonary sarcoidosis.</p><p>A total of 253 persons were under study including 122 patients diagnosed with morphologically verified sarcoidosis with lung involvement (average age, 41.00±12.56 years), and 131 healthy donors comprising a control group (average age, 44.00±14.23 years). The distribution of alleles and genotypes for the Arg753Gln (rs574308) polymorphic TLR2 gene marker was studied in the groups of patients with pulmonary sarcoidosis and healthy donors.
