При многих системных ревматических заболеваниях, сопровождающихся поликлональной гиперреакивностью В-лимфоцитов, наблюдаются изменения в В-клеточных субпопуляциях. У пациентов с системными ревматическими заболеваниями иммуносупрессивная и цитостатическая терапия также избирательно влияет на субпопуляции В-клеток. Мы исследовали субпопуляции В-клеток у больных системными ревматическими заболеваниями во время их лечения цитостатиками. Мы проанализировали В-клеточные фенотипы у 99 больных с системными ревматическими заболеваниями: 25 с системной красной волчанкой, 27 с системной склеродермией , 47 с синдромом Шегрена , проходивших лечение в стационаре. Группу контроля составили 49 здоровых доноров. Измерение фенотипов В-клеточных субпопуляций проводилось с помощью проточного цитофлуометра (Beckman Coulter, США). «Наивные» В-клетки у больных cистемной красной волчанкой, получавших циклофосфан, были снижены по сравнению с группой контроля, в то время как плазмобласты были повышены, независимо от терапии. В-клеточная популяция является гетерогенной по своей природе, именно поэтому каждую В-клеточную субпопуляцию при системных ревматических заболеваниях необходимо рассматривать как самостоятельную функциональную единицу. Несмотря на проводимую иммуносупрессивную терапию у пациентов с системной красной волчанкой, уровень плазмобластов, активно вырабатывающих антитела, оставался высоким. Таким образом, терапия, направленная на определенные B-клеточные субпопуляции, способна увеличить эффективность лечения больных системными ревматическими заболеваниями.</p></sec><sec><title> </title><p> </p></sec></abstract><trans-abstract xml:lang="en"><p>Distinct changes of B-cell subpopulations are observed in most systemic rheumatic diseases associated with polyclonal B cell hyperreactivity. Immunosuppressive and cytostatic therapy may also differentially influence B lymphocyte subsets in these. We studied subpopulations of B cells in systemic rheumatic patients along treatment with cytostatics. We analyzed B cell phenotypes in ninety-nine blood samples from the patients with systemic lupus erythematosus (SLE, n = 25), systemic sclerosis (n = 27), Sjogren’s syndrome (n = 47) in the course of their hospital treatment. Control group consisted of 49 healthy blood donors. Phenotyping of blood B-cell subpopulations was performed by means of flow cytometry (Beckman Coulter, USA). Naïve B-cell subpopulations in SLE patients who underwent cyclophosphan treatment, were underrepresented, if compared with normal control group, whereas plasmablast levels were increased irrespectively of medication mode. B cell population exhibits a natural heterogeneity, thus making it necessary to analyze distinct B cell subpopulations as independent functional units, when studying different rheumatic diseases. The levels of plasmablasts which are active antibody producers, remain high, despite immunosuppressive therapy performed in SLE.
